Make a Referral

Introductory Program: When the Speech-Language Pathologist has reviewed all reports and completed the screening process, the candidate/family will be notified, and an appointment for our assessment/ Introductory Program will be scheduled. This is a prerequisite to becoming a HARC participant. The Introductory Program includes a 3-4 hour individualized assessment process and additional training for caregivers and participants in Supported Communication for Adults with Aphasia.

Candidacy: Candidates with aphasia referred to HARC will be required to participate in an initial tour of our facility and meet with one of our Speech-Language Pathologists. At that time, eligibility will be determined. Additional medical records may be requested. Candidates may bring a completed Release of Information form to the initial tour/meeting.
