Health professionals may make a referral to HARC by:

• Completing an online referral
• Calling 713-781-7100. Ask to speak to a Speech-Language Pathologist for more information. 
• Faxing a completed HARC referral form to 713-781-7105. Include a copy of a discharge evaluation from a licensed Speech-Language Pathologist, Neuropsychologist or the Applicant’s Rehabilitation Physician.
